COVID-19 CLINIC POLICY

1. HAVE YOU TRAVELLED (DOMESTIC / INTERNATIONAL) IN THE LAST 4 WEEKS?
2. HAVE YOU HAD RECENT CONTACT WITH A KNOWN OR SUSPECTED COVID-19 PATIENT IN THE PAST TWO WEEKS?
3. DO YOU RESIDE IN OR HAVE YOU VISITED A HIGH-RISK AREA WITH A CLUSTER OF CASES?
4. HAVE YOU RECENTLY TESTED FOR COVID19?
5. DO YOU HAVE ANY OF THE FOLLOWING?:
• Temperature > 37.2°C
• A cough
• A sore throat
• Shortness of breath
• Other respiratory symptoms
• A recent loss of sense of smell

PLEASE NOTE UPON ARRIVAL TO OUR CLINICS
• Patients MUST use our hand sanitiser on arrival
• Temperature checks on arrival
• Please conform to social distancing requirements
• Gloves, masks and tissues are readily available
